Josie Johnson Memorial Ride Jersey Order Form

We are asking for a minimum $50 donation to the Utah Bicycle Coalition for the Jersey and a $10 donation for a Tee Shirt.

Enter your information below, once we have enough orders, we will contact you with payment details and confirm the order via e-mail.

We hope to place an order by November 11th 2005 and then will require 6 weeks for delivery. It's possible (but not guaranteed) they will be available by Christmas. Checks will not be cashed until the order is placed.

You will be informed of the order and receipt of the new shirts by email.

We will place the shirts at Spin Cycle (4644 S Holladay Blvd) where you can pick them up. Please allow an additional $5 to each order for Priority Mail shipping if you can't pick-up.
